I would try re-install from the disk (doing a full complete install) then install 445 patch then install each level one at a time - single player levels first. Some levels need additional mods such as the sky common database pack. This can be found on the downloads page of this site.

Is the level you are unzipping a self extracting .zip (.exe) or just a normal .zip file? If you are using a self extracting one that error will appear if the directory already exists.
